Cortica is looking for a remote Nurse Practitioner to join its growing team. In this role, you will conduct thorough patient intakes, screenings, and assessments, considering the unique needs of each patient and their family. You will triage referrals for acute needs and collaborate with interdisciplinary team members to coordinate comprehensive care. By utilizing the nursing process, scientific reasoning, and critical problem-solving skills, you will practice within the State Board of Nursing scope, placing orders and screening tests in accordance with approved protocols and guidelines. Your responsibilities will also include providing accurate clinical documentation using the EMR system, effectively communicating with patients, families, and clinical teams, and participating in annual performance reviews. Additionally, you will welcome new families via telehealth visits, share the overall care model, and help families access appropriate developmental and behavioral services while gathering information about neurodevelopmental challenges and conducting diagnostic assessments and screenings to facilitate further referrals for therapy and medical services.
