The Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) practices in Surgery Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. Duties unique to Wound Care include but are not limited to: Performing thorough evaluations of wounds related to diabetes, vascular disease and poor circulation including size, depth, location, tissue viability, and presence of infection. Creating individualized care plans based on the wound assessment, including appropriate dressings, medications, and therapies. Performing procedures like debridement, wound irrigation, etc. Providing detailed instructions to patients and caregivers on wound care techniques, hygiene practices, and preventative measures to promote healing. Consulting with physicians, other nurses, and specialists to coordinate comprehensive patient care. Regularly assessing wound healing and making necessary adjustments to the treatment plan as needed. Identifying patients at high risk for developing pressure ulcers and implementing preventative strategies. Performs History and physical for pre-op patients. Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
