About The Position

The Nurse Practitioner (VSP) applies advanced education and clinical competencies to achieve optimal patient outcomes. The Nurse Practitioner (VSP) works on problems of diverse scope and complexity ranging from moderate to substantial. As a Nurse Practitioner (VSP), you will apply your advanced education and clinical competencies to achieve optimal patient outcomes. You will work on problems of diverse scope and complexity ranging from moderate to substantial. You will work as a variable service provider NP/PA (APP) role with an average of 20-30 hours per week for a 6-month period. You will work in a remote environment, reporting to our Associate Director, Advanced practice Provider. There are no benefits tied to this position.

Requirements

  • Master's degree or higher
  • Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners Certification Board (AANPCB)
  • Active, unrestricted NP license in 2-3 or more of the following states: FL, TX, AZ, GA, LA, NC, SC, MS, VA prior to the start of employment
  • Strong communication and interpersonal skills, allowing you to build collaborative relationships with patients, families, and care teams.
  • Ability to excel in an integrated, team-based care environment, adapt to value-based care practices, and remain focused on delivering high-quality outcomes.
  • Commitment to exceptional patient service, compassion, and a patient-centered approach is essential.
  • Self-provided internet service with at least a download speed of 25 Mbps and an upload speed of 10 Mbps; wireless, wired cable or DSL connection is suggested.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Nice To Haves

  • Previous experience in a value-based care or Medicare Advantage setting
  • Knowledge of HEDIS measures and quality indicators
  • Experience conducting annual wellness or developing comprehensive care plans for patients who may only be seen once
  • Experience in virtual care and remote work

Responsibilities

  • Conducting virtual (audio and video) visits with CenterWell patients to care for chronic conditions, discuss open care gaps, and connect patients back to an in-person PCP.
  • Address acute and chronic conditions and preventive care needs.
  • Creating and registering, and documenting a patient record within Athena
  • Verify benefits
  • Comprehensive patient evaluation including medication and medical history review.
  • Coordinating management of members social needs to help facilitate gap closure – e.g., providing transportation services
  • Ensure adequate follow up regarding receipt of results, medication initiation, and PCP follow up
