The Nurse Practitioner - Substance Use Health is a key member of the Mental Health and Addictions Treatment (MHAT) Hub, providing client-centered, trauma-informed, and culturally safe primary health care to individuals experiencing complex challenges related to substance use, homelessness, and mental health. Working within an interdisciplinary team, the Nurse Practitioner provides comprehensive assessments, treatment, and health promotion across the MHAT services, including the MHAT Hub, 24/7 residential treatment, mobile, drop-in, outreach, street health, and collocated services. This role ensures equitable and accessible care by addressing the social determinants of health and implementing innovative, community-based approaches to primary care delivery.
