This position exists to promote excellence in clinical nursing practice by providing expertise in advanced clinical practice, education, consultant services, and clinical research. The Nurse Practitioner will assess health status through obtainment of relevant health history and comprehensive physical exam, preventive screening, and identification of medical risk factors. They will formulate appropriate differential diagnosis and identification of individual, family and community needs, and develop treatment plans to maximize the health potential of the individual, family, and/or community. The role may involve patient care conferences, facilitating discharge planning, and developing policies and procedures. Ordering and adjusting medications per collaborative written agreement and prescriptive authority in accordance with the Indiana Nurse Practice Act, or in consultation with a physician, is also a key responsibility. The Nurse Practitioner will direct the development, implementation, and evaluation of standards for nursing practice, and serve as a clinical resource for nursing and other back office staff, physicians, and other health care providers. They will also act as an advocate for patients, staff, and management, working with a diverse population including those of low socioeconomic status, multi-cultural/multilingual backgrounds, with substance use, low educational attainment, and late and sporadic care patients. The role emphasizes advancing clinical skills and knowledge through continuing medical education, collaborating on staff education programs, and promoting wellness and self-care by sharing information for healthy lifestyle choices and health care decisions. Applying at least one health literacy method when communicating with patients/clients is required. The Nurse Practitioner will function as a clinical expert, providing consultative services to health team members and the community, and marketing these services to problem-solve complex clinical situations. Promoting and participating in continuous improvement principles to enhance the effective utilization of resources and collaborating with leadership on quality improvement initiatives are also essential. Comparing clinical practices to national standards to positively affect patient outcomes is expected.
