This role involves consulting with and educating patients and their families regarding their disease process, self-care techniques, and end-of-life care. The Nurse Practitioner will also consult with and assist in the Interdisciplinary Group's (IDG) development of a patient/family plan of care to ensure quality care. Collaboration with attending and consulting physicians regarding palliative care, in conjunction with the hospice physician, is a key responsibility. The practitioner will inform physicians and other personnel of changes in patient needs and intervention outcomes, while evaluating patient/family response to care. Timely submission of all documentation and performance of other duties as required are expected. Ensuring overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures is also part of the role.
