This is a full-time Nurse Practitioner position focused on primary care for older adults. Requirements

• CA Nurse Practitioner license
• 1 year experience as a nurse practitioner in a geriatric setting/ working with the frail or elderly is required.
• Current active DEA license

Responsibilities

• Conducts physical exams, diagnostic tests, blood draws, EKGs, dressing changes, and other nursing interventions.
• Assesses patients' conditions, formulates treatment plans including prescriptions, and provides ongoing follow-up.
• Educates staff, participants, and caregivers on healthcare issues and treatments.
• Manages acute and long-term medical needs in collaboration with other providers, covering care in various settings including nursing homes and assisted living facilities.
• Provides emotional and spiritual support, manages medical emergencies, and ensures timely, compliant documentation in medical records.
