About The Position

We are seeking a compassionate, knowledgeable, and dedicated Nurse Practitioner (NP) to join our healthcare team. In this role, you will play a vital part in delivering high-quality, patient-centered care across the continuum. The Nurse Practitioner independently manages and coordinates patient care within the scope of licensure and applicable State and Federal regulations. This position combines clinical expertise, collaboration, patient education, and advocacy to help individuals achieve better health outcomes and navigate their healthcare journey with confidence.

Requirements

Master's Degree in Nursing.
Current prescriptive authority.
Minimum of 5 years of nursing experience.
Current, active Nurse Practitioner license in the State of Ohio.
Valid driver's license and current automobile insurance.
Excellent written and verbal communication skills.
Strong interpersonal and customer service skills.
Demonstrated clinical judgment and strong medical aptitude.

Responsibilities

Manage and coordinate the overall care of assigned patients, providing comprehensive, compassionate, and evidence-based care.
Assess, diagnose, and treat acute and chronic health conditions, including injuries, infections, diabetes, hypertension, and other medical concerns.
Develop and implement individualized treatment plans based on patient needs, clinical findings, and established standards of care.
Prescribe medications and treatments within the NP's scope of practice and applicable prescribing authority.
Monitor changes in patient condition and determine appropriate follow-up care, treatment modifications, referrals, or additional appointments.
Provide patients and families with clear after-care instructions, ongoing care resources, and education regarding preventive health and prescribed treatments.
Perform initial patient screenings, obtain comprehensive health histories, and conduct physical examinations.
Facilitate new patient consultations and establish therapeutic relationships with patients and families.
Perform invasive procedures and administer medications in accordance with established clinical standards and policies.
Safely and effectively operate medical equipment and instruments.
Order, conduct, analyze, and interpret diagnostic testing, including laboratory studies, X-rays, EKGs, and other appropriate diagnostic procedures.
Provide high-quality, physician-level clinical care while adhering to agency policies, safety requirements, clinical standards, and productivity expectations.
Maintain accurate, complete, and up-to-date patient records at every encounter.
Ensure medical records include current test results, immunizations, diagnoses, treatment plans, and relevant changes in patient condition.
Collaborate with physicians, nurses, allied health professionals, and other members of the multidisciplinary care team to coordinate comprehensive patient care.
Make appropriate referrals to specialists and other healthcare providers when indicated.
Help patients successfully transition between healthcare settings, including admission, transfer, and discharge.
Serve as a knowledgeable resource for patients and families navigating ongoing or complex healthcare needs.
Provide counseling, guidance, and support to patients experiencing critical or complex health conditions.
Educate patients and families using appropriate tools and resources to support informed healthcare decisions.
Advocate for the needs, preferences, and well-being of individuals, families, groups, and communities.
Promote preventive care, healthy behaviors, treatment adherence, and effective self-management.
Support patients and families in understanding their conditions, treatment options, medications, and ongoing care needs.
Provide clinical oversight, delegation, and direction to RNs, LPNs, and Medical Assistants within applicable scopes of practice.
Promote consistent use of established procedures, policies, clinical guidelines, and professional standards.
Foster a collaborative, respectful, and patient-focused team environment.
Perform other duties and responsibilities as assigned.
