Nurse Practitioner - Primary Care
About The Position
Become a part of our caring community We are looking for a compassionate and patient-centered Nurse Practitioner to join our primary care clinic focused on delivering high-quality, value-based care to a senior population. This role is ideal for an NP who is passionate about geriatrics, preventive medicine, and building long-term relationships with patients. You will work as part of an interdisciplinary care team in a supportive, clinic-based environment designed to allow providers to focus on patient care—not volume. You will report directly to the Associate Medical Director.
Requirements
- Active, unrestricted Nurse Practitioner license in the state of practice
- National certification (AANP or ANCC)
- DEA license (or ability to obtain)
- Experience in primary care, family medicine, internal medicine, or geriatrics preferred
- Comfort working with Medicare and managed care populations
- Strong communication skills and a patient-focused approach to care
Responsibilities
- Provide comprehensive primary care services to adult and geriatric patients, including assessments, diagnosis, treatment, and ongoing care management
- Manage chronic conditions with an emphasis on preventive care and wellness
- Perform physical exams, order and interpret diagnostic tests, and develop individualized treatment plans
- Collaborate closely with physicians, care coordinators, behavioral health specialists, and clinical support staff
- Focus on quality-driven outcomes, including HEDIS measures, CMS Star initiatives, and population health goals
- Educate patients and caregivers on disease management, medications, and healthy lifestyle choices
- Maintain accurate and timely documentation in the electronic medical record (EMR)
- Participate in care team meetings and quality improvement initiatives
