Nurse Practitioner – Primary Care East Jefferson General Hospital | Metairie, Louisiana Join a trusted healthcare leader and make a lasting impact in the community. East Jefferson General Hospital, part of LCMC Health, is seeking a compassionate, patient-focused Primary Care Nurse Practitioner to join our growing outpatient team. This full-time opportunity is ideal for an advanced practice provider who is passionate about preventive medicine, chronic disease management, and building meaningful relationships with patients across the continuum of care. As a Primary Care Nurse Practitioner, you will collaborate with physicians and an interdisciplinary care team to deliver high-quality, evidence-based care while helping patients achieve their best health outcomes. Requirements

  • Master's Degree from an accredited Nurse Practitioner Program required
  • Current Registered Nurse (RN) license issued by the Louisiana State Board of Nursing
  • Current Advanced Practice Registered Nurse (APRN) license issued by the Louisiana State Board of Nursing
  • Basic Life Support (BLS) certification through the American Heart Association

Responsibilities

  • Deliver Exceptional Patient Care
  • Perform comprehensive health assessments, physical examinations, and preventive health screenings
  • Diagnose and manage acute and chronic health conditions
  • Develop individualized treatment plans and evaluate patient responses to care
  • Educate patients and families on disease prevention, treatment options, and healthy lifestyle choices
  • Respond promptly to patient calls, messages, and clinical concerns
  • Maintain accurate and timely clinical documentation within the electronic medical record
  • Recognize and respond appropriately to medical emergencies
  • Identify and report signs of abuse, neglect, or other patient safety concerns in accordance with organizational policy
  • Collaborate & Lead
  • Work closely with physicians, nurses, clinical staff, and leadership to ensure coordinated patient care
  • Participate in daily huddles, provider meetings, and quality improvement initiatives
  • Serve as a clinical resource and leader for the care team
  • Contribute to practice operations and support a culture of continuous improvement
  • Ensure Compliance & Quality
  • Adhere to all federal, state, and regulatory requirements
  • Maintain patient confidentiality and HIPAA compliance
  • Stay current with continuing education and professional development requirements
  • Support quality, safety, infection prevention, and performance improvement initiatives
