About The Position

The kind of primary care you’d want for your own family — delivered to theirs. At Medical House Calls, you won’t rush through a waiting room full of strangers. You’ll walk into your patients’ homes, sit down at their kitchen tables, and take care of entire families; kids, parents, grandparents, as their one consistent provider. You’ll know their names, their histories, and your patients’ dogs. And you’ll have the time to actually practice. We’re looking for a Family Nurse Practitioner to manage a panel of primary care families across Middle Tennessee, delivering the kind of unhurried, relationship-driven care that patients tell their friends about.

Requirements

  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) required.
  • Active, unrestricted Nurse Practitioner license in the state(s) of practice required.
  • Family Nurse Practitioner (FNP) certification required.
  • 2–5 years of Nurse Practitioner experience required, in family practice, primary care, or urgent care, with experience managing a continuing patient panel.
  • Current Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification preferred.
  • Active Drug Enforcement Administration (DEA) registration required, or obtained upon hire where applicable.
  • Valid driver’s license, reliable transportation, and current auto insurance meeting Company requirements.

  • Advanced Cardiovascular Life Support (ACLS) certification preferred.

Responsibilities

  • Serve as the consistent provider for an assigned panel of primary care families, building continuity of care over time.
  • Deliver longitudinal primary care across the household — preventive and wellness visits, chronic disease management, and acute care as it comes up.
  • Conduct evaluations, physical exams, and comprehensive health assessments.
  • Diagnose, build individualized care plans, and prescribe and administer medications and treatments within your scope.
  • Perform minor in-home procedures consistent with scope of practice and MHC clinical protocols.
  • Order, interpret, and clearly communicate diagnostic and lab results to patients and families.
  • Coordinate care across the broader team, including specialist referrals and follow-up on outside consults.
  • Document encounters accurately and on time in the EHR.
  • Maintain compliance with collaborative practice, supervising physician, and chart review requirements in each state you practice.
  • Participate in on-call or extended-hours coverage as scheduled.
  • Serve as a clinical resource to the MHC team and support onboarding of new providers as we grow.
  • Deliver an experience that keeps families with us — and gets us referred to their neighbors.
