About The Position

Ascend Medical is seeking a compassionate and experienced Nurse Practitioner or Physician Assistant to provide relationship-centered care at premier Senior Care communities in Sugar Hill and Grayson, GA. This part-time role is ideal for a provider who is passionate about geriatrics and values quality time with patients over quantity. You’ll be supported by a dedicated clinical care team, innovative technology, and full operational support so you can focus on what matters most—delivering exceptional care and building trusted relationships. Requirements

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Georgia (unrestricted, non-probationary).
  • Master’s degree from an accredited program.
  • Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health background.
  • Active DEA license (or eligibility to obtain).
  • Availability to work on-site in Sugar Hill and Grayson; occasional visits to additional locations may be required.

Responsibilities

  • Provide compassionate, relationship-based care to older adults in an assisted living environment.
  • Build trust with patients, families, and facility staff through clear communication and consistent presence.
  • Manage chronic conditions, address acute concerns, and tailor care plans to the unique goals of each patient.
  • Collaborate with the facility’s care team to ensure seamless, coordinated support.
  • Apply your home health or hospice experience to improve outcomes for medically complex patients.
  • Maintain flexible availability for patient visits and on-site presence.
