The University of Rochester's Complex Care Center (CCC) provides person-centered and coordinated interdisciplinary care to adolescents and adults with complex pediatric onset conditions. These conditions include, but are not limited to, intellectual/developmental disabilities, sickle cell disease, cystic fibrosis, cerebral palsy, and muscular dystrophy. The CCC offers interdisciplinary clinical services such as primary care, dentistry, physical/respiratory therapies, dietician, and behavioral health. It also houses URMC’s adult cystic fibrosis and adult sickle cell disease programs. This role involves direct patient care, treating acute illnesses and complex chronic diseases, including performing complete history and physicals, follow-up office visits, coordinating diagnostic testing, and prescribing medications. New hires will receive APP mentoring and support through a structured orientation/onboarding program.
