About The Position

Salud Family Health is seeking a full-time Nurse Practitioner or Physician Assistant in Trinidad, Colorado. Since 1970, Salud has worked to provide a coordinated Medical Home, integrating medical, dental, behavioral health, and clinical pharmacy services for patients, regardless of insurance status or ability to pay. Providers use a team-based care approach to support patients and families in self-management, self-efficacy, and behavior change. The role includes routine clinic operations, taking patient histories, and performing physical assessments in keeping with Salud’s mission.

Requirements

Graduated from, or be graduating from, an accredited Nurse Practitioner or Physician Assistant program.
Hold an unrestricted Nurse Practitioner or Physician Assistant license in good standing in Colorado.
Demonstrate sensitivity to low-income and ethnic minority communities.

Nice To Haves

Ability to speak Spanish is strongly preferred.
Three years of clinical-setting experience as a Nurse Practitioner or Physician Assistant is preferred.

Annual salary of $118,100 to $151,850.
