Nurse Practitioner/Physician Assistant
About The Position
Total Extended Care Services, a growing Physical Medicine and Rehabilitation group, is seeking a dedicated Nurse Practitioner or Physician Assistant to work alongside a PM&R physician in a sub-acute rehabilitation setting. The role focuses on improving patient outcomes in senior living communities as part of an interdisciplinary, specialized team. The position is in person at skilled nursing facilities, with flexible full-time and part-time scheduling options. Timely charting and documentation are required and may be completed from home. Requirements
- Licensed Nurse Practitioner.
- Certified Nurse Practitioner.
- Licensed Physician Assistant.
- Certified Physician Assistant.
- Must be licensed in Ohio.
- Reliable transportation.
Responsibilities
- Partner with the facility’s therapy department to provide the best possible rehabilitation care.
- Communicate and collaborate with the patient’s primary care team to support the best outcomes.
- Complete charting and documentation in a timely manner; this work may be done from home.
