Primary Care Network (Canalside Family Medicine) Nurse Practitioner / Physician Assistant

This is an exciting opportunity for an experienced NP or PA to join a busy Family Medicine practice and become part of an interdisciplinary team that provides patient care utilizing the Patient Centered Medical Home model. This is a fast-paced office in the Rochester suburb of Greece, NY area. The providers care for newborns, children, teenagers, and adults across all ages and medical issues. Ultimately, the APP will have an established team supported patient panel. An APP mentoring program is available for all new hires along with thoughtful onboarding. The candidate must possess NYS NP/PA certification along with licensure and have applied for DEA registration prior to hire/start date. NPs and PAs will demonstrate clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn. The APP will participate in the education of learners (students and new employees), using clinical learning experiences, knowledge sharing and skill training.
Requirements
- NYS NP/PA certification
- NYS NP/PA licensure
- Applied for DEA registration prior to hire/start date
- Master's degree as a Nurse Practitioner (for NPs)
- Active New York State Registered Nurse license in good standing (for NPs)
- Active New York State Nurse Practitioner licensure in good standing (for NPs)
- Current national certification as an Nurse Practitioner (for NPs)
- Completion of an ARC-PA approved Physician Assistant program (for PAs)
- Active New York State Physician Assistant licensure in good standing (for PAs)
- Current national certification as a Physician Assistant by NCCPA (for PAs)
- Demonstrate clinical competence
- Superior verbal and written communication skills
- Interpersonal effectiveness and customer relation skills
- Computer competence
- Active desire to learn

- Greater than 2 years of active primary care practice

Responsibilities
- Evaluation and management of acute/chronic illnesses
- Ordering/interpreting/communicating diagnostic tests and results
- Complete physical exams
- Wellness visits
- Hospital/ED follow-ups and Transitional Care Management
- Collaboration with network based physicians and medical team regarding patient care
- Facilitating appropriate referrals to specialists or community-based agencies
- Opportunities for office based procedures
- Patient education/counseling intended to maintain or restore physical/emotional health and function
- Educational support for various learners consistent with the goals of the UR teaching institution
