About The Position

UR Medicine Cardiac Care, Regional Cardiology is seeking a highly motivated Advanced Practice Provider (NP/PA) to serve as an integral member of the general cardiology team. This role provides comprehensive cardiovascular care across outpatient and inpatient settings, primarily in the Canandaigua office, with time in Geneseo and Clifton Springs outpatient offices as necessary. The position emphasizes independent management of patients in the outpatient cardiology clinic, with additional responsibilities supporting inpatient rounding in collaboration with attending cardiologists. Requirements

  • Master's degree as a Nurse Practitioner (for NP role)
  • Active New York State Registered Nurse license in good standing (for NP role)
  • Active New York State Nurse Practitioner licensure in good standing (for NP role)
  • Current national certification as an Nurse Practitioner (for NP role)
  • Completion of an ARC-PA approved Physician Assistant program (for PA role)
  • Active New York State Physician Assistant licensure in good standing (for PA role)
  • Current national certification as a Physician Assistant by NCCPA (for PA role)

Responsibilities

  • Independently evaluate, diagnose, and manage patients with a broad range of cardiovascular conditions, including but not limited to hypertension, heart failure, coronary artery disease, valvular heart disease, arrhythmias, and dyslipidemia.
  • Perform comprehensive history and physical examinations, order and interpret diagnostic studies (ECG, echocardiography, stress testing, ambulatory monitors, laboratory work), and develop evidence-based treatment plans.
  • Manage chronic disease states including guideline-directed medical therapy titration for heart failure, anticoagulation management, and cardiovascular risk factor optimization.
  • Provide pre- and post-procedural evaluation and follow-up for patients undergoing cardiac catheterization, electrophysiology procedures, or cardiac surgery.
  • Counsel patients and families on disease management, lifestyle modification, medication adherence, and preventive cardiology.
  • Triage clinical calls, patient messages, and results.
  • Coordinate referrals to cardiac subspecialties (electrophysiology, interventional cardiology, advanced heart failure, cardiac surgery) and other medical disciplines.
  • Collaborate with attending cardiologists to round on hospitalized cardiology patients at Thompson Hospital, contributing to daily assessment, care planning, and documentation.
  • Communicate with multidisciplinary teams including nursing, pharmacy, case management, and consulting services to ensure coordinated, high-quality inpatient care.
  • Contribute to discharge plan and continuity of care.
  • Maintain timely, accurate, and thorough documentation in the electronic health record.
  • Participate in quality improvement initiatives, departmental meetings, and peer review activities.
  • Engage in continuing medical education and maintain board certification and state licensure.
  • Contribute to the education of trainees, nursing staff, and other learners as appropriate.
