Nurse Practitioner / Physician Assistant

MedMatchNovi, MI

About The Position

Join a physician-owned practice caring for adults 18 and older in our Novi office and for older, medically complex patients in homes and senior communities. This is longitudinal primary care: you become a trusted clinician for patients and families, rather than completing one-time assessments. The role is primarily weekdays, with shared telephone coverage and no acute-care hospital rounding. Clinical breadth and support: Combine adult primary care with geriatrics, dementia care, behavioral health and geriatric behavioral medicine, chronic pain management, and palliative and hospice coordination. Use individualized medication optimization, including careful reduction of controlled substances when clinically appropriate. Provide prevention, acute assessment, chronic disease management and goals-of-care conversations, with timely escalation when a patient needs a higher level of care. Work directly with the physician-owner for case consultation and mentorship, supported by clinical and administrative teammates and CGM APRIMA training.
