About The Position

Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join Community Care Internal Medicine – Delmar, a well-established internal medicine practice dedicated to providing comprehensive, evidence-based care for adult patients. POSITION OVERVIEW As an Advanced Practitioner at Community Care Internal Medicine – Delmar, you will provide adult primary care in a collaborative, team-based environment alongside board-certified internists and experienced Advanced Practice Providers. The practice emphasizes preventive medicine, chronic disease management, and accessible care supported by modern technology and strong specialty integration. PRACTICE HIGHLIGHTS Adult-focused internal medicine practice – caring for patients ages 18+ with an emphasis on long-term health and chronic disease management Flexible access model, including early morning and late appointment availability for both routine and sick visits Same-day appointment availability for time-sensitive patient needs Telemedicine and secure patient portal, supporting convenient communication and continuity of care Located within a multispecialty health park, offering on-site access to imaging, lab, physical therapy, podiatry, and more for seamless coordination Strong, collaborative team environment with multiple physicians and Advanced Practitioners serving a well-established patient panel On-call coverage model, ensuring continuous patient support while maintaining outpatient focus

Requirements

  • License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
  • Physician Assistant: Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
  • Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
  • Current certification by National Commission on Certification of Physician Assistants.
  • At least one year of experience as a physician assistant in ambulatory health care.
  • Current CPR certification.
  • Nurse Practitioner: Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
  • Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
  • Successful completion of pharmacology coursework acceptable to NYSED.
  • At least one year of experience as a nurse practitioner in ambulatory health care.
  • Current CPR certification.

Nice To Haves

  • Three years of professional nursing experience in an ambulatory setting preferred.

Responsibilities

  • Conduct comprehensive evaluations for adult patients and manage acute and chronic conditions
  • Order and interpret diagnostic studies, prescribe medications, and coordinate specialty referrals
  • Provide preventive care, annual wellness visits, and patient education
  • Collaborate closely with physicians, Advanced Practitioners, and support staff to ensure continuity of care
  • Maintain accurate, timely clinical documentation in accordance with organizational standards
