About The Position

Valley Sleep and Wellness Center (VSWC) is seeking to hire a Part Time Nurse Practitioner/Physician Assistant to participate in the rewarding work of assisting physicians with delivering primary health care to our patients. VSWC specializes in offering clinical services that treat both adult and pediatric sleep disorders in the Central San Joaquin Valley. Requirements

  • Completion of an accredited school of nursing
  • Possession of State Registered Nurse license
  • National certification
  • 3 or more years of professional clinical experience in a clinic setting is preferred
  • Candidates should have exceptional people and communication skills, be highly skilled in multi-tasking in a fast-paced environment, a commitment to achieving a high level of accuracy and attention to detail, and a no-task-too-small approach to the work.

Responsibilities

  • Obtain complete medical history and physical data on patients
  • Collaborate with physicians in managing acute and long-term medical needs of patients
  • Interpret and integrate data and order laboratory studies and other diagnostic and therapeutic procedures as indicated
  • Review lab and sleep studies results, in consultation with the supervising physician as indicated
  • Submit patient health care plan and goals of individual patients for periodic review and evaluation by physician
  • Refer patient to physician or specialist for consultation
  • Review consultation reports, in consultation with the supervising physician as indicated
  • Counsel patients and their families on health promotion, diagnosis, prognosis and management options. Recommend community resources to meet patient and family needs.
  • Instruct patient and authorized family members regarding medications and treatment instructions. Provide patient education.
  • Maintain medical record in the problem-oriented format as per standard company/site practice
  • Triage patient telephone calls and provide consultation
  • Provide monitoring and continuity of care between physician visits
  • Attend required meetings as requested
  • Participate in professional development and maintain professional affiliations
  • Maintain strictest patient confidentiality in compliance with Federal and State laws
