This is a full-time (40 hours) Nurse Practitioner (NP) or Physician Assistant (PA) position with 10-hour shifts, primarily on weekdays, with occasional weekend and holiday coverage per Division policy. It offers an exciting opportunity to join an adult Palliative Care inpatient practice within a large academic medical center. The service attends to patients admitted to the Palliative Care Unit (4-1200) for end-of-life care and provides consultations elsewhere in the hospital for patients with serious illnesses at all stages. The goals are to manage complicated symptoms, facilitate care goals conversations, and provide support to patients, families, and colleagues. This role includes cross-training and time spent in the Emergency Department's embedded palliative care program and the inpatient advanced heart failure Palliative Care program. The APP's primary responsibility is the collaborative care of patients and families, encompassing consultative and admission work-ups, follow-up visits, team rounding, EHR chart documentation, order writing for 4-1200 patients, and discharge planning. Additional responsibilities include participation in clinical and educational conferences and rounds, precepting APP students, teaching and supporting multidisciplinary trainees, and assisting nurses, clinicians, and staff on 4-1200 and other hospital units.
