Nurse Practitioner/Physician Assistant

Altais includes a network of physician-led organizations across California. They are looking for a Nurse Practitioner to join a fast-growing, dynamic team. This role involves performing comprehensive health assessments, including obtaining history, physical examinations, reviewing medicines, assessing home safety and psychosocial needs, fulfilling documentation for STAR and HCC measures, and obtaining/documenting pertinent patient information. The Nurse Practitioner will also instruct patients and families on when to access medical care, maintain patient safety and confidentiality, utilize universal precautions, prescribe/order procedures, treatments, and medications within their scope of practice, and develop a high level of trust and intimacy with patients and families.
Requirements
- CA Licensed Nurse Practitioner
- 1+ Years of Experience with Primary Care
Responsibilities
- Perform comprehensive health assessment
- Obtain history and perform physical examination
- Complete review of medicines and related instructions
- Review home safety and psychosocial needs
- Fulfill documentation needs for STAR and HCC measures
- Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions
- Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies
- Maintain patient safety and confidentiality
- Universal precautions utilized in all patient care
- Prescribe/order procedures, treatments and medications within scope of practice
- Develop high level of trust and intimacy with patients and families
