About The Position

Evolutionary Healthcare is seeking compassionate and motivated Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our growing team providing primary care, pain management, and collaborative Osteopathic Manipulative Treatment (OMT) services throughout post-acute and skilled nursing facilities in Central Florida. Requirements

  • Current Nurse Practitioner or Physician Assistant license in Florida (or ability to obtain)
  • National Board Certification
  • DEA license (or ability to obtain)
  • Excellent clinical judgment and communication skills
  • Commitment to compassionate, patient-centered care

Nice To Haves

  • Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, Emergency Medicine, or Post-Acute Care preferred
  • Skilled Nursing Facility experience is a plus, but not required

Responsibilities

  • Provide comprehensive primary care services in skilled nursing and post-acute care facilities.
  • Evaluate, diagnose, and manage acute and chronic medical conditions.
  • Collaborate with physicians providing Osteopathic Manipulative Treatment (OMT) services.
  • Identify patients who may benefit from OMT and coordinate referrals.
  • Develop individualized treatment plans that improve function, mobility, and quality of life.
  • Perform routine follow-up visits, admissions, discharge planning, and transitional care.
  • Collaborate closely with nursing staff, therapists, specialists, and facility leadership.
  • Educate patients and families regarding disease management and preventive care.
  • Maintain accurate and timely documentation using electronic health records.
