Primary Care Network – Artemis (Brighton, NY) – Nurse Practitioner / Physician Assistant
About The Position
The University of Rochester is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join the Artemis Health Internal Medicine Practice in Brighton, NY. This role is part of a well-established interdisciplinary team that utilizes the Patient Centered Medical Home model to provide care for adult patients over 18. The NP/PA will have the opportunity to establish a team-supported patient panel and will be an integral part of a teaching institution, contributing to the education of learners.
Requirements
- At least 1-2 years of experience, preferably in Primary Care
- NYS NP/PA certification
- NYS NP/PA licensure
- Applied for DEA registration prior to hire/start date
- Master's degree as a Nurse Practitioner (for NPs)
- Active New York State Registered Nurse license in good standing (for NPs)
- Active New York State Nurse Practitioner licensure in good standing (for NPs)
- Current national certification as an Nurse Practitioner (for NPs)
- Completion of an ARC-PA approved Physician Assistant program (for PAs)
- Active New York State Physician Assistant licensure in good standing (for PAs)
- Current national certification as a Physician Assistant by NCCPA (for PAs)
- Demonstrate clinical competence
- Superior verbal and written communication skills
- Interpersonal effectiveness and customer relation skills
- Computer competence
- Active desire to learn

Responsibilities
- Evaluation and management of acute/chronic illnesses
- Ordering, interpreting, and communicating diagnostic tests and results
- Completing physical exams
- Conducting Wellness visits
- Managing hospital/ED follow-ups and Transitional Care Management
- Collaborating with network-based physicians and the medical team regarding patient care
- Facilitating appropriate referrals to specialists or community-based agencies
- Performing office-based procedures
- Providing patient education and counseling to maintain or restore physical/emotional health and function
- Providing educational support for various learners consistent with the goals of the UR teaching institution
