About The Position

We are looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to round on residents at assisted living and board and care facilities in Central and North Orange County. This role involves primary care, managing chronic conditions, handling acute changes in condition, and preventing hospital transfers by getting to know patients well. The model emphasizes spending adequate time at the bedside to truly understand patient needs.

Requirements

• Licensed NP or PA in good standing.
• Comfortable in an ALF or board and care setting, or ready to learn.
• Open to SNF exposure if of interest.
• Solid clinical judgment.
• Comfortable communicating with facility staff, families, and interdisciplinary teams.
• Valid driver's license and a reliable car.

Nice To Haves

• Background in geriatrics, internal medicine, family practice, home health, hospice, or ALF/SNF/board and care/LTC work.
• New grads are welcome.

Responsibilities

• Rounding on a consistent panel of ALF and board and care residents, plus some SNF and home visits depending on territory and interest.
• Managing chronic disease, med changes, and acute issues on-site to help residents and facilities avoid unnecessary ER trips.
• Performing thorough visits including history, exam, care planning, and family and staff communication.
• Working alongside facility staff and Brigade Health's psych, palliative care, and neurology teams.
• Keeping documentation current in the EMR.
