About The Position

Our Float Pool team is comprised of highly skilled medical professionals who are trained to seamlessly integrate into our primary care practices and help maintain continuity of patient care. Float Pool providers support our practices by filling staffing gaps due to unexpected absences, planned vacations, extended leaves, and vacant positions. We are seeking a per diem Nurse Practitioner or Physician Assistant who can commit to providing at least 24 patient-facing days per calendar year. Providers will submit their availability in advance, and assignments will be scheduled based on operational needs across our North Shore practices. In addition to providing direct patient care, the NP or PA will be responsible for managing follow-up on patient results and in-basket messages related to their clinical assignments for up to two weeks after each shift, ensuring continuity of care for pending tests, results, and patient communications. The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations. May also perform additional duties, such as precepting a small group of learners.

Requirements

  • Master's Degree Nursing required
  • Advanced Practice Registered Nurse [APRN-NP - State License] - Generic - HR Only required
  • 2 years of licensed nurse practitioner experience
  • 1-2 years required
  • Prior experience as a Nurse Practitioner or Physician Assistant in a Primary Care setting is required.

Responsibilities

  • Provides direct care, counseling, and education to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
  • Performs complete histories and physical examinations.
  • Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
  • Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
  • Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
  • Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
  • Performs bedside procedures as are appropriate to the patient population.
  • Managing follow-up on patient results and in-basket messages related to their clinical assignments for up to two weeks after each shift, ensuring continuity of care for pending tests, results, and patient communications.
