About The Position
The APP will provide comprehensive medical care, symptom management, and emotional support, working to enhance comfort, promote healing, and improve quality of life for every patient throughout their recovery journey. The APP will support patients and families as they transition from hospital to post-acute settings, managing recovery and complex health needs with expertise and empathy.
