This role involves managing health problems and coordinating care for Oncology/Hematology patients, conducting comprehensive patient assessments, developing and implementing care plans, and collaborating with the healthcare team. The position requires travel to float locations in Smithtown and Port Jefferson.

Requirements

Graduate of an accredited Nurse Practitioner or Physician Assistant program.
Current Advanced Practice Registered Nurse (APRN) Licensure or Physician Assistant License.
Strong computer skills; experience with EMR is a plus.
Must have reliable transportation to travel to all locations listed.

Nice To Haves

At least one (1) year of experience in Oncology/Hematology.

Responsibilities

Manage health problems and coordinate care for Oncology/Hematology patients in alignment with State and Federal regulations and nursing standards of care.
Conduct comprehensive patient assessments, develop and implement care plans, and evaluate patient status.
Collaborate with the healthcare team to prepare patients preoperatively and manage postoperative clinical care.
Deliver patient-centered care focused on improving outcomes and enhancing the patient experience.
