The Nurse Practitioner/Physician Assistant will be apart of the Care Anywhere (CAW) program is designed to provide care and support to our Alignment Healthcare patients by providing an additional level of medical and social support in the comfort of their own home. This program is offered to eligible patients at no cost to them with the goal of preventing unnecessary hospitalizations, health complications, and unmanaged disease progression that can occur when timely clinical interventions are not provided or are not accessible.

Requirements

  • Master’s degree from an accredited NP Program or PA program
  • Ability to communicate positively, professionally and effectively with others.
  • Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment.
  • Active Nurse Practitioner or Physician Assistant license
  • NP only Active Nurse Practitioner Board Certification/Physician Assistant Certification
  • NPI Number, DEA, Valid BLS
  • Valid Driver License and current automobile insurance

Nice To Haves

  • One (1) year of prior clinical or home care experience.
  • Previous EMR experience preferred.
  • Experience in care of older adult (geriatric) patients preferred

Responsibilities

  • M-F Traditional Work Hours, No Call, and No Weekends
  • Schedule: 3.5 days a week conduct in-home assessments (with some outpatient clinical work) on health plan members (Max 5 per day) and the other 1.5 days a week work from home.
  • Identify diagnoses to be assessed in care management and active medical management
  • Partner and communicate with Regional Medical Officer and other team members to discuss and develop the most appropriate care plans possible based on the needs of our members/patients.
  • Coordinate care with multiple stakeholders, including but not limited to PCPs, specialists, and ancillary providers.
  • Lead broader clinical team which may include nurses, health coaches, social workers, and care coordinators
