About The Position

The Advanced Heart Failure APP is an advanced practice clinician who performs acts of evaluation, diagnosis, and treatment in collaboration with attending physicians to manage the health outcomes of patients with Advanced Heart Failure. As an advanced practice provider this role involves independent judgment and decision-making as well as consultation and collaboration with other members of the health care team. The Advanced Heart Failure APP enhances the continuity of quality patient care; amplifies communication between the physician, family members, and the health care team; and extends the capabilities of the medical care team. This position is critical in leading quality and evidence-based care initiatives that will result in achieving optimal patient outcomes and improved patient satisfaction for the Advanced Heart Failure patient population.

Requirements

  • Completion of a Nurse Practitioner or Physician Assistant program required.
  • MSN required.
  • Licensed by the Pennsylvania State Board of Nurse Examiners as a Practitioner, or by the Pennsylvania State Board of Medical Education and Licensure.
  • Certification by the NCCPA (National Commission for Certification of Physician Assistants) or (CRNP) Certified Registered Nurse.
  • BLS certification required.
  • Successful completion of the MLH credentialing process through the Medical Staff office, with temporary or permanent privileges approved.
  • Two (2) years' of Cardiology experience as a Nurse Practitioner or Physician Assistant Required.

Nice To Haves

  • Advanced Heart Failure experience highly preferred.
  • VAD (Ventricular Assist Device) Coordinator experience highly preferred.
  • May require ACLS certification.
  • May require PALS (pediatric advanced life support)

Responsibilities
