Part-Time Nurse Practitioner Palliative Medicine (Outpatient)
About The Position
Samaritan is seeking a diligent and compassionate Nurse Practitioner for its outpatient Palliative Medicine team. The Nurse Practitioner will work with the Palliative Medical Partners group to provide consultation and follow-up visits for patients in outpatient settings, including patients’ homes, care facilities, and clinics. This part-time role involves collaborating with patients, families, primary care physicians, attending physicians, consulting physicians, and specialists to provide patient- and family-centered care. The Nurse Practitioner will develop and maintain internal and external relationships and work with support staff and other team members to meet patient and partner needs and coordinate care across Samaritan’s programs. The schedule is Monday through Friday, 28 hours per week, with an after-hours telephone-call rotation; the rotation does not require visits.
Requirements
- RN and APN licenses in New Jersey.
- DEA and CDS licenses are required.
- Obtain palliative certification within two years of hire.
Nice To Haves
- Prior experience in palliative care, oncology, heart failure, geriatric care, and/or hospice is preferred.
Responsibilities
- Provide a health assessment appropriate to each visit.
- Diagnose and treat patients who need symptom management and goals-of-care conversations.
- Collaborate with patients, families, attending physicians, and other specialists to provide patient- and family-centered care.
- Develop and maintain internal and external relationships while working with support staff to meet the needs of internal and external partners.
- Coordinate care with other team members across Samaritan’s family of programs.
- Participate in an after-hours telephone-call rotation; calls do not require visits.
