About The Position

Provides clinical expertise, assists in clinical development, working collaboratively within an interdisciplinary care team, ensuring standards of care and providing consultation and expertise in the delivery of patient care. Activities include consultation as requested, assistance in case conferencing, clinical supervision, provision of patient care and clinical over site for Hope Hospice and assisting the medical director and associate medical director in conducting patient visits including face to face encounters. Provide palliative care for identified patients in their homes and designated health facilities. Consult and assist the referring physician and other members of interdisciplinary team to coordinate the patient's plan of care. Provide consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the referring physician.

Requirements

  • Current Texas License as an RN.
  • Authorization to practice as an Advanced Practice Nurse Program (APN) in the state of Texas. This may include Interim Approval, Provisional Authorization, or Full Authorization to Practice according to the Texas Board of Nurse (BON) Examiners Advance Practice Nurse Regulations Rule 221.
  • Limited Prescriptive Authority by Texas BON Examiners and Texas Board of Medical Examiners.
  • Master’s degree in nursing or related field.
  • Existing National Certification as a Nurse Practitioner. This certification may be pending successful completion of the national exam.
  • Strong communication and assessment skills.
  • Reliable transportation, proof of automobile insurance.

Nice To Haves

  • Preferred specialty area(s) include one or more of the following: Adult Nurse Practitioner, Gerontological Nurse Practitioner, Family Nurse Practitioner, Acute Care Practitioner, or Psychiatric/Mental Health Practitioner.
  • A National Certification in Hospice and Palliative Care (CHPN) is preferred, but not required.
  • Certification in Education for Physicians in End-of-Life Care (EPEC) preferred.
  • Hospice or Palliative care experience preferred.

Responsibilities

  • Provides clinical expertise and oversight for Hope Hospice in conjunction with the Hospice Medical Director and Hospice Associate Medical Directors.
  • Conducts daily rounds on existing and new patients and completes visit documentation under the supervision of Hospice Medical Director and Associate Medical Director.
  • Completes discharge and death summaries.
  • Works collaboratively with the Director of Patient Care Services and other team members to ensure effective day-to-day operations within the teams, ensure the care provided to patients meets agency and regulatory standards, is of the utmost quality, is effective and efficiently uses resources.
  • Provides leadership to visiting staff to ensure appropriate multidisciplinary communication, collaborative working relationships, and a customer-oriented environment responsible to the needs of external/internal customers.
  • Is readily available to visit with staff for consultation, problem solving, and support.
  • Ensures adherence to Hospice Conditions of Participation.
  • Participates in orientation of new staff, acting as a resource, supporting the preceptors, and identifying opportunities for staff development for new orientees as well as all team members.
  • Ensures excellent patient care services, coordinating educational and outreach services, participating in quality improvement activities, and linking efforts with inpatient and community services.
  • Identifies opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resource use.
  • Assists in the development and review of appropriate clinical protocols, policies, and processes that support appropriate referrals and care.
  • Develops and delivers appropriate educational initiatives within the agency or other settings.
  • Provides physician back up and coverage responsibilities where appropriate including writing prescriptions i.e., Comfort Care Kit.
  • Shares Hospice on call responsibilities with Hospice Medical Director, Associate Medical Directors, and Nurse Practitioners.
  • Assists Hospice Medical Director and Associate Medical Director in conducting hospice patient home visits, palliative care consultation visits and face to face encounter visits.
  • Determine a plan of care specific to each patient’s needs.
  • Prepare and maintain accurate patient records, charts, and documents to support the medical practice and reimbursement for services provided.
  • Foster and maintain relationships with hospital-based palliative care teams, medical directors, hospital physicians, community physicians, nurses, and other team members as well as community members to build a culture of collaboration and clinically integrated care across the continuum.
  • Proactively identify opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resource use throughout.
  • As the subject matter expert, educate, support, and assist Hope Hospice team members, the community, and interested groups.
  • Attend monthly Palliative Care team meetings to review active cases.
  • Participate in periodic team meetings for the operational and administrative functions of the program.
  • Be available on an as needed basis to support other team members with clinical advice and assist with physician communication needs.
  • Be available on an as needed basis to support educational and outreach goals of the program.
  • Act as spokesperson for the Palliative Care Program.
  • Support philosophy and objectives of Hope Hospice.
  • Continue professional competency in area of expertise.
  • Maintain professional licensure and/or certification; know, understand, and adhere to rules and guidelines.
  • Stay current with governmental and accreditation requirements affecting area of responsibility.
