About The Position
A growing palliative care organization is seeking an experienced Nurse Practitioner to provide high-quality patient care while helping lead and mentor a team of advanced practice providers across New Jersey. This hybrid role combines direct patient care with clinical leadership and offers the opportunity to make a meaningful impact on patients living with serious illnesses and complex chronic conditions.
Requirements
- Active New Jersey Nurse Practitioner license
- National board certification and DEA registration
- Minimum 5 years of clinical experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or chronic disease management
- Strong communication and leadership skills
- Experience working within interdisciplinary care teams
Nice To Haves
- Leadership or supervisory experience
- Hospice & Palliative Care certification (ACHPN)
- Experience with home-based care, telehealth, or value-based care models
- Bilingual skills are a plus
Responsibilities
- Provide palliative care assessments and follow-up visits
- Manage patients with serious illness and complex chronic conditions
- Facilitate goals-of-care and advance care planning discussions
- Collaborate with physicians, specialists, hospitals, and community providers
- Mentor and support Nurse Practitioners and clinical staff
- Participate in quality improvement and operational initiatives
- Assist with onboarding, training, and team development
