About The Position
The Nurse Practitioner, Palliative Care provides comprehensive palliative care services to clients, ensuring high-quality and professional care. The role includes conducting visits with new and established palliative care patients and coordinating their care plans within the designated team. Visits take place in patients’ homes, facilities, and hospitals. The Nurse Practitioner makes independent decisions regarding patients’ healthcare needs and delivers palliative care services as required. The Medical Director is available for consultation, collaboration, and referrals as needed.
Requirements
- Graduation from an accredited Baccalaureate school of nursing.
- Completed training in the diagnosis and management of common and some complex medical conditions.
- Current licensure in the State of California.
- Certification as an Adult, Family, Pediatric, or Women’s Health Nurse Practitioner.
- Previous experience with computer-based systems.
- A valid BLS certification or higher, following American Heart Association or American Red Cross guidelines.
Nice To Haves
- Experience with electronic medical records.
- Previous experience in hospice or palliative care, high-risk populations, or home-based care.
- Bilingual in Spanish and English.
