About The Position

We are seeking an experienced Nurse Practitioner with exceptional communication and relationship-building skills to provide independent palliative care services, focusing on symptom management, goals-of-care discussions, and patient-centered support in the home setting. This role serves patients throughout Delaware and Montgomery Counties, allowing you to make a meaningful impact where patients are most comfortable. As a Nurse Practitioner you will perform acts of medical diagnosis or prescription of medical therapeutic or corrective measures in collaboration with attending physicians and residents. You will make an impact in leading quality and evidence-based care initiatives that will result in achieving optimal patient outcomes.

Requirements

  • Completion of a Nurse Practitioner program required.
  • MSN Required.
  • Licensed by the Pennsylvania State Board of Nursing Examiners as a Certified Registered Nurse Practitioner.
  • Provider BLS (American Heart Assoc/American Red Cross) required.
  • Require ACLS certification.
  • Successful completion of the MLH credentialing process through the Medical Staff office, with temporary or permanent privileges approved.

Nice To Haves

  • Previous home care, hospice, or palliative care experience is preferred.
  • 2 years Nurse Practitioner experience preferred in palliative home care.
  • Minimum of 2 years Nursing experience in palliative home care preferred.
  • May require PALS (pediatric advanced life support).

Responsibilities

  • Provide independent palliative care services in the home setting.
  • Focus on symptom management, goals-of-care discussions, and patient-centered support.
  • Perform acts of medical diagnosis or prescription of medical therapeutic or corrective measures in collaboration with attending physicians and residents.
  • Lead quality and evidence-based care initiatives to achieve optimal patient outcomes.
  • Amplify communication between the physician, family members, and the health care team.
