PACE Primary Care Providers (PCPs) deliver comprehensive, patient-centered primary care to a medically complex, frail elderly population. This role includes managing chronic diseases, addressing acute care needs, providing preventive services, and overseeing medication management. PCPs work closely with the interdisciplinary team (IDT) to coordinate care, develop individualized care plans, and ensure timely documentation and follow-up based on biannual and as-needed assessments. In addition to clinical responsibilities, PCPs actively contribute to quality improvement initiatives aimed at reducing hospitalizations and emergency department utilization, improving risk adjustment accuracy, closing preventive care gaps, and enhancing chronic disease outcomes.
