About The Position

This role involves providing daily healthcare services to patients in a skilled nursing facility. Responsibilities include managing patient schedules, conducting patient visits, completing electronic charting, ordering medications and treatments, and participating in on-call duties and provider meetings.

Responsibilities

Review schedule for assigned facility.
Review scheduled appointments, check with Nursing Director for acute patient needs, and check with Medical Director for other needs.
Add any new identified patient visits to the schedule and organize the day according to scheduled visits.
Visit patients as scheduled, complete appropriate charting during the visit, and communicate with facility staff as needs arise.
Prescribe and approve new medications, approve refill medications, order and approve labs, and order and approve new treatments as needed.
Complete electronic charting on patients within 24 hours of the visit.
Sign each chart within 24 hours of each visit.
Create a charge for each visit within 24 hours of each visit.
Complete daily visit log for each patient, including name, date, and CPT code for each visit.
Send visit log to the business office within 24 hours.
Participate in on-call duties as assigned.
Complete on-call log and send it to the business office within 24 hours of completed on-call duties.
Participate in weekly provider meetings.
Submit hours, mileage, and PTO requests on a weekly basis to the business office.
