Requirements

• Master’s Degree in Nursing from an accredited school.
• Current certification from the American Academy of Nurse Practitioners (ANNP) or American Nurses Credentialing Center (ANCC).
• Current license or ability to obtain licensure from the SC Board of Nursing (NP).
• Minimum of five years general medical experience in inpatient, outpatient or nursing home settings.
• Minimum two years experience as a nurse practitioner required.
• Demonstrated competence in leadership and teaching.

Nice To Haves

• Certification in hospice/palliative care preferred.
• Experience seeing patients in a home setting preferred.

Responsibilities

• See patients in various venues of care including personal residences, assisted living facilities, skilled nursing facilities, as well as clinic and telehealth visit support.
• Educate caregivers on how to care for their loved ones, including disease education, advance care planning, medication administration, and end-of-life expectations.
• Spend extended time driving between patients, documenting, and coordinating care.
• Coordinate care between patient’s community medical providers and other disciplines to provide updates and obtain new orders.
• Participate in an Interdisciplinary Team (IDT) consisting of a physician, nurse practitioner, registered nurse, and a medical social worker, attending IDT education rounds and palliative care staff meetings.
