About The Position

The Nurse Practitioner (NP) is an expert in diabetes care. The NP is responsible for the delivery of diabetes care and education that involves compliance with evidence-based clinical standards, outcomes measurement (tracking, analysis, and reporting of clinical, financial, and satisfaction outcomes), and process design (leading initiatives to facilitate practice-based and organizational change). As a member of the glycemic management team, the NP provides direct patient care in the outpatient and community setting through implementation of standardized procedures, collaboration with the medical team to establish priorities, and assessment of solutions for patients to receive exceptional care. The NP functions under general Nurse Practitioner Standardized Procedures approved by the Committee on Advanced Practice Providers (CAPP) and reports to the Diabetes Services Program Manager.

Requirements

  • Master’s degree in nursing required (exception NP qualified or certified as an NP in California or any other state prior to January 1, 2008)
  • Relevant NP training/certification
  • State of California Board of Registered Nursing licensure and authorization as a NP required
  • Certified by a recognized national certifying body that has established standards for NPs
  • Furnishing license issued by the Board of Registered Nursing is required
  • American Heart Association Healthcare Provider BLS certification is required
  • Current valid driver’s license and functional transportation are required

Nice To Haves

  • Clinical background in diabetes management is preferred
  • Two (2) years in outpatient diabetes services preferred
  • Experience in managing insulin pumps preferred
  • Spanish fluency preferred
  • Certified Diabetes Care and Education Specialist (CDCES) credential and Board Certified-Advanced Diabetes Management (BD-ADM) preferred upon hire and must be completed within 24 months from hire date
