About The Position

Humana's Primary Care Organization is seeking a full-time Primary Care Nurse Practitioner or Physician Assistant to join our team at our clinic in Daytona Beach, FL. As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, diverse team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more, all working together to deliver personalized, high-quality care. The role focuses on improving healthcare and lives, including the employee's. The position involves a team-based care model, allowing more time with patients, and fosters a supportive, inclusive culture where teamwork and growth are prioritized.

Requirements

• Master's degree in nursing or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Experience in value-based care, primary care or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Provide value-based care.
• Work on-site.
