About The Position

Impact Health is looking for an experienced Nurse Practitioner or Physician Assistant to provide weekend clinical coverage in our Fairfield, CT Territory. Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, dedicated to prioritizing patient care. We specialize in Medical, Psychiatric Services, Wound Care and Palliative Care. This role offers a flexible schedule with no nights, weekends, or holidays, and provides a collaborative environment that values expertise and independence.

Requirements

• Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
• Minimum 2 years of post-certification experience
• Proficient with EHRs and clinical technology
• Strong communicator with a collaborative mindset

Nice To Haves

• Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred

Responsibilities

• Deliver person-centered care using evidence-based practices
• Collaborate with interdisciplinary facility teams
• Conduct assessments, develop treatment plans, and manage follow-up care
• Educate patients, families, and staff
• Complete timely documentation (within 24 hours)
• Ensure compliance with state regulations in SNF settings
