Our relationship-based model combines remote patient monitoring and proactive clinical engagement to keep patients healthier and out of the hospital.Complex conditions like COPD are incredibly challenging to manage. Requirements

  • Active Nurse Practitioner or Physician Assistant license in the state of Tennessee.
  • Minimum of 1–2 years of clinical experience in pulmonary medicine, internal medicine, family medicine, or a related specialty.
  • Comfort with outpatient care delivery models.
  • Strong clinical judgment, communication skills, and patient-centered approach to care.
  • Ability to work collaboratively within a multidisciplinary team.
  • Comfortable working in an electronic medical record (EMR).

Nice To Haves

  • Prior experience in pulmonary, respiratory, or chronic disease management.
  • Familiarity with remote patient monitoring, Telehealth platforms, or digital health tools.
  • Experience reviewing PFTs, imaging, and managing long-term respiratory conditions.

Responsibilities

  • Provide outpatient pulmonary and primary care to adult patients with conditions such as COPD, asthma, chronic cough, and other respiratory disorders.
  • 1-2 days/week (flexible Monday-Thursday) approx 7:45a-4:30pm
  • Conduct patient evaluations, follow-up visits, and care coordination in collaboration with supervising pulmonologists.
  • Review pulmonary diagnostics, including spirometry, PFTs, imaging (e.g., chest X-rays), and other relevant clinical data.
  • Manage treatment plans, medication adjustments, and patient education related to pulmonary disease management.
  • Collaborate with respiratory therapists, nurses, care coordinators, and clinical support staff.
  • Document care accurately and efficiently within the electronic health record.
  • Adhere to all clinical protocols, quality standards, and regulatory requirements.
