About The Position

We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our clinical team in an Inpatient Rehabilitation Facility (IRF) setting. In this role, you will conduct daily patient rounds, provide acute and transitional care management, and collaborate closely with physiatrists, therapy teams, and interdisciplinary staff to support optimal functional recovery and safe discharge planning for patients undergoing intensive rehabilitation.

Requirements

  • Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
  • Masters Degree in Nursing or Physician Assistant Studies
  • Must possess excellent written and oral communication skills
  • Must possess traits of punctuality, organizational skills, and ability to function within a team.
  • This role requires travel to and from one or more assigned facilities.

Responsibilities

  • Conduct structured daily rounds on assigned IRF patients, performing focused assessments, monitoring rehabilitation progress, and addressing acute and subacute medical needs.
  • Manage comorbidities and complications common in the IRF population, including post-stroke recovery, orthopedic post-op care, traumatic brain injury, cardiac conditions, and neurological disorders.
  • Partner with physiatrists, physical therapists, occupational therapists, speech-language pathologists, case managers, and nursing staff to develop and execute coordinated, patient-centered care plans.
  • Actively participate in discharge planning, including assessment of functional status, coordination with case management, and communication with receiving facilities or home health agencies.
  • Write and manage orders for medications, diagnostics, therapies, and consults; respond promptly to nursing and therapy team communications throughout the day.
  • Evaluate and manage acute changes in patient condition, initiate appropriate interventions, and determine when escalation or transfer is warranted.
  • Maintain thorough, timely documentation in the EMR consistent with IRF regulatory standards, including required physician/APP attestation requirements and CMS compliance guidelines.
  • Communicate regularly with patients and families regarding medical status, goals of care, rehabilitation expectations, and discharge planning.
  • Participate in quality improvement initiatives focused on reducing hospital transfers, improving functional outcomes, and supporting IRF performance benchmarks.
