About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships. The goal is to provide flexible, comprehensive, and personalized health evaluations. The visit, lasting about 45 minutes, includes a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The position is part of a national network of clinicians dedicated to improving health outcomes and connecting people to the right care.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification.

Responsibilities

Conduct In-Home Health Evaluations.
Engage with people in the comfort of their own homes.
Bridge gaps in care.
Build trusted relationships.
Provide flexible, comprehensive, and personalized health evaluations.
Conduct medication and medical history reviews.
Perform physical evaluations.
Conduct additional diagnostic tests if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
Travel locally and within licensed states, as needed.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
