About The Position

Signify Health, part of CVS Health®, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role requires flexibility to travel locally and within licensed states, and may include virtual visits.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification (required).
Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.
Able to cover the primary locations: Yakima County and surrounding area, Yakima MSA, Yakima County.

Responsibilities

Conduct In-Home Health Evaluations.

Conduct medication and medical history reviews.
Perform physical evaluations.
Conduct additional diagnostic tests if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
Travel locally and within licensed states, as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products.
Conduct virtual visits as needed.
