As a Nurse Practitioner or Physician Assistant in Physician Services, you will practice medicine in a way that prioritizes time, connection, and meaningful patient relationships. You will provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on delivering high-quality, patient-centered care. This role is ideal for someone who values flexibility, collaboration, and a supportive clinical environment. You will benefit from no weekends, holidays, or on-call requirements, a flexible schedule, and a limited patient census. Strong back-office support, streamlined credentialing, structured onboarding, and ongoing collaboration with physicians and peers will be provided. In this role, you will conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. You will serve as a key clinical decision-maker while helping patients and families understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations.

Requirements

  • Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant
  • Current State License as a Nurse Practitioner or Physician Assistant
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Nice To Haves

  • Minimum 2 years clinical experience in a post-acute or as a Nurse Practitioner or Physician Assistant
  • Adult, Family, or Gerontology certification
  • Experience with geriatric patients

Responsibilities
