About The Position

Liberty Surgical Services is seeking a Nurse Practitioner or Physician Assistant for their Inpatient division at the Liberty location. This role will cover 7 divisions: Pediatric Surgery, Gastroenterology, Plastic Surgery, Urology, Gynecology, Orthopedics, and ENT. The APP will manage inpatients admitted directly, from the OR, or from the ED, and will complete all Inpatient and ED Consults for Pediatric Surgery, Urology, and Gynecology. The role involves performing history and physical exams, assessing and diagnosing illnesses, formulating and monitoring treatment plans, ordering and monitoring diagnostic tests, providing clinical consultation, and prescribing medications in accordance with all applicable laws and policies. The position requires maintaining competency in subspecialty practice, skills, and procedures, and participating in quality improvement activities and lifelong learning. The APP will also serve as a preceptor, role model, or mentor, advocate for patients and families, and potentially influence healthcare policy. Care management responsibilities include promoting self-management and family-centered care, employing complex interventions, documenting and communicating with the healthcare team, delegating tasks, and making appropriate referrals. Evaluation of patient care is systematic and ongoing, involving patients, families, and other providers, and utilizing results to recommend process or structural changes. The scope of practice for this subspecialty role will not exceed that of the collaborating or supervising physician.

Requirements

  • Nurse Practitioner and/or Physician Assistant
  • At least 1-2 years of experience preferred
  • Certification-FNP, Acute Care, Primary Care, etc.
  • Master's degree in related field.
  • May require clinical license, certification or training OR Doctor of Nursing Practice degree
  • Licensure and certification in accordance with CCHMC Delineation of Privileges
  • Multi-state licensure in addition to Ohio licensure may be required based on subspecialty

Nice To Haves

  • Experience in a surgical division or GI preferred
  • Experience as an APP in subspecialty

Responsibilities

  • Perform history and/or physical exams on patients presenting for care in area of expertise.
  • Assess and diagnose illnesses, including chronic and acute conditions.
  • Formulate, document and monitor a treatment plan.
  • Order and monitor therapeutic interventions including but not limited to diets, laboratory, radiological and diagnostic tests as deemed necessary by patient history and/or physical exam.
  • Provide clinical consultation, order interdisciplinary consults.
  • Prescribe medications in accordance with Federal and State Law & Rules and CCHMC policy.
  • Attain and maintain competency in subspecialty practice, skills and procedures in accordance with Joint Commission standards for focused (FPPE) and ongoing (OPPE) professional practice evaluation.
  • Develop outcomes as measurable goals that provide direction for continuity of care & consistency among providers.
  • Develop, prioritize, evaluate & modify an individualized comprehensive plan of care.
  • Address each diagnosis or issue, as appropriate.
  • Derive outcomes & plans of care that involve the patient/family/caregiver when possible & are realistic to the patient/family/caregiver capabilities & resources.
  • Consider risks/benefits, costs, clinical effectiveness, current scientific evidence/clinical guidelines & clinical expertise to formulate interventions, treatments & outcomes.
  • Incorporate new knowledge & strategies to initiate change in practice if desired outcomes not achieved.
  • Maintain documentation & updates accordingly outcomes, goals & the plan of care.

  • Delegate appropriate monitoring, assessment, & interventions according to the patient and scope of practice of the caregiver.
  • Provide consultation to influence the identified plan of care, to enhance the abilities of others to provide health care, and to effect change in the healthcare system.
  • Make appropriate referrals and discuss recommendations with patient/family/caregiver.
  • Order and/or perform appropriate treatments, therapies, & procedures that are based on current knowledge, research, & practice.
  • Provide patient/family/caregiver with diagnostic & laboratory results as well as actual/potential benefits, limitations, & adverse effects of proposed therapies.
  • Use a systematic, ongoing evaluation of the assessment data & outcomes to revise or resolve the diagnoses, outcomes, the plan of care, & the implementation as needed.
  • Evaluate the accuracy of the diagnosis, effectiveness of the interventions & the plan of care strategies in relation to attainment of expected outcomes.
  • Include the patient/family/caregiver & other healthcare providers in the evaluation process.
  • Base the evaluation process on advanced knowledge, practice, research, & document appropriately.
  • Utilize the results of the evaluation to determine the impact on the patient/family/caregiver, organization, & system to make/recommend process or structural changes including policy, procedure, or protocols as appropriate.
