About The Position

Impact Health PC is hiring a Nurse Practitioner / Physician Assistant in Dover, NH. We are a leading advanced practitioner group in skilled nursing facilities across the Northeast, dedicated to prioritizing patient care. We offer flexible employment opportunities with multiple scheduling options, including Full Time, Part Time, and PRN. This role is ideal for an experienced NP or PA seeking a flexible, well-supported role in a collaborative environment that values expertise and independence, with a Monday through Friday schedule and no nights, weekends, or holidays.

Requirements

• Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
• Minimum 2 years of post-certification experience
• Proficient with EHRs and clinical technology
• Strong communicator with a collaborative mindset

Nice To Haves

• Experience in SNFs, hospitalist roles, emergency departments, or ICU/SICU preferred

Responsibilities

• Deliver person-centered care using evidence-based practices
• Collaborate with interdisciplinary facility teams
• Conduct assessments, develop treatment plans, and manage follow-up care
• Educate patients, families, and staff
• Complete timely documentation (within 24 hours)
• Ensure compliance with state regulations in SNF settings
