About The Position

Signify Health, part of CVS Health®, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests like diabetic eye exams or spirometry if ordered by the health plan. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, or other airborne allergens.

Requirements

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Able to cover the following primary locations: Marquette (Counties: Alger, Baraga, Marquette)
Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.

Responsibilities

Conduct In-Home Health Evaluations.
Engage with individuals in their homes to bridge gaps in care.
Build trusted relationships to improve health outcomes.
Perform medication and medical history reviews.
Conduct physical evaluations.
Perform diagnostic tests such as diabetic eye exams and spirometry, if ordered.
Use an iPad and an intuitive clinical workflow for each evaluation.
Travel locally and within licensed states, as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products.
Conduct virtual visits as needed.
