About The Position

Theoria Medical is seeking an APP (FNP, NP, or Adult Gero NP) or PA based in Agawam, MA, to serve local skilled nursing facilities within a close-knit, mission-driven care community. Our network includes multispecialty provider services, covering skilled nursing facilities across the country. This role emphasizes building relationships, consistency, and presence, allowing you to care for patients over time, building trust, strengthening continuity, and making a lasting difference for vulnerable populations. The model offers flexible scheduling and autonomy, supported by intuitive technology and a collaborative clinical team.

Requirements

  • Active U.S. license and board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants)
  • Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
  • Must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.
  • Theoria Medical conducts criminal background checks and pre-employment drug testing on all candidates upon acceptance of contingent offer.

Responsibilities

  • Provide high-quality, patient-centered care to residents in skilled nursing facilities
  • Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
  • Communicate with facility nursing staff via phone or secure messaging as needed
  • Maintain reliable on-site attendance for scheduled shifts
  • Ensure compliance with clinical standards, facility policies, and regulatory requirements
